How to create a survey form

To create a survey form you need to

  1. Go to ‘'Management’' section, then click on ''Forms

  2. Click on Survey Form on the left corner

  1. Click on ‘'+ Add’' button, fill in the information and click the ‘'OK’' button

  1. Write the Title and types of questions that you want.

  1. You can view the forms you've already created in the "Request" section under Management.
